If incorrect information was entered during a high school assessment, you may need to request a change so the wrestler's assessment record is accurate. Assessment changes require review, so you'll submit a change request with the necessary documentation to your state office.

Submit an assessment change request

Use the High School Season Assessment Change Request Form to request a correction to assessment data.

Your documentation will be sent to your state office for review.

Submit one request per team. For example, if an entire team's assessments failed because of late entry, submit one request for the team instead of an individual request for every wrestler.

When not to use the assessment change request

Do not use the assessment change request form for:

  • Physician clearance: Contact your assessor if a wrestler needs physician clearance because of low body fat.
  • Transferring a wrestler to a new school: Transfers require approval from the state governing body.
  • Changing a wrestler's gender: Submit a help ticket for assistance.
  • Correcting a typo in a wrestler's name: Submit a help ticket for assistance.
  • Assessment appeals: Contact your state office for its specific appeal procedures.
